How to obtain Ozempic and Mounjaro legally in Spain. The legitimate routes, the risks of buying without a prescription, real prices and what to look for in a serious medical service.

An important warning before you start
If you have come to this article looking for how to buy Ozempic or Mounjaro without a prescription, there is something you should know before reading on: in Spain, selling these medicines without a medical prescription is illegal. And the websites that offer them without a prescription are not only operating outside the law: in all likelihood they are selling counterfeit or badly stored product, or referring you to services that issue prescriptions irregularly.
This article explains the genuine legal routes to access these medicines, the specific risks of buying without a prescription, official prices, and the criteria to apply when choosing a legitimate medical service. The information has been reviewed by Dr David Céspedes, Medical Director at AiON.
The legal framework for dispensing in Spain
In Spain, both Ozempic (semaglutide) and Mounjaro (tirzepatide) are prescription-only medicines. That classification means:
- Dispensing them without an official prescription is illegal.
- The prescription must be issued by a doctor registered with a Spanish medical council, with all the regulatory details required.
- Dispensing must take place exclusively in authorised community pharmacies.
- Selling prescription medicines by post or online is prohibited under Spanish Law 10/2013.
This regulation exists to protect public health. GLP-1 agonists have specific contraindications and significant side effects, they require individualised dose titration, and they can interact with other medical conditions. That is why they are not consumer products.
Legal routes to Ozempic and Mounjaro in Spain
If a doctor considers these treatments to be indicated in your case, there are three legitimate routes to access them.
Route 1: the Spanish National Health System
This applies only to patients with type 2 diabetes who meet the inclusion criteria set out in the National Health System (SNS) service catalogue. Your primary care doctor or the endocrinologist within the public system assesses your case and, if appropriate, issues a subsidised electronic prescription. The patient pays only the co-payment corresponding to their income level.
An important limitation: the Spanish public health system does not currently cover use for weight loss without diabetes. For obesity alone, the patient has to go private.
Route 2: a private face-to-face appointment with an endocrinologist
The patient attends a private appointment with an endocrinologist. The doctor carries out a full clinical assessment: history, examination, up-to-date blood tests, review of contraindications, treatment goals. If they consider the treatment appropriate, they issue a private electronic prescription that can be dispensed at the pharmacy of the patient's choice.
Typical cost of this route:
- Initial appointment with a private endocrinologist: 120 to 300 euros.
- Follow-up appointments: 80 to 200 euros.
- Medication at the pharmacy: 120 to 280 euros a month, depending on the drug and the dose.
Route 3: private telemedicine with a registered doctor
An emerging option that allows an online medical assessment with a doctor registered with a Spanish medical council, the issuing of a private electronic prescription, and collection of the medicine at the patient's usual pharmacy.
This route is entirely legitimate provided that specific conditions are met, which we look at below. Telemedicine is regulated and recognised as a valid medical act in Spain, and a private electronic prescription has exactly the same legal standing as one issued in a face-to-face appointment.
What you should NOT do under any circumstances
Some practices may look like shortcuts to getting hold of these medicines, but they carry serious risks, both legal and medical.
Buying online without a prescription
Any website offering Ozempic, Mounjaro or any other GLP-1 agonist without requiring a medical prescription is operating illegally. The product may be:
- Counterfeit: uncertain contents, incorrect doses, adulterated ingredients.
- Expired or badly stored: GLP-1 agonists require a strict cold chain.
- Obtained through illegal diversion from the official supply chain.
In 2023 and 2024, the Spanish medicines agency (AEMPS) issued several alerts about counterfeit Ozempic pens circulating in Europe, with serious consequences for the patients affected, including cases of severe hypoglycaemia caused by adulterated contents.
Importing from other countries without a prescription
Bringing prescription medicines in from abroad without a valid prescription in Spain is also illegal. Foreign online pharmacies that ship to Spain operate in a grey area or are directly in breach of European rules.
Using unauthorised "compounded" formulations
In the United States, a parallel market has grown up for compounded (specially prepared) versions of semaglutide and tirzepatide, exploiting legal loopholes during periods of shortage. These formulations are not authorised or evaluated by either the AEMPS in Spain or the FDA, and the FDA has already sent warnings to companies marketing them with misleading claims. In Spain they have no legal basis for use whatsoever.
Buying from private individuals
Buying from people who "resell" medication left over from their own prescription is illegal and dangerous. There is no guarantee of proper storage, correct dosing or legitimate origin.
Real pharmacy prices in Spain
To give you an idea of the real cost of treatment, these are the usual price ranges with a private prescription at a Spanish pharmacy as at the date this article was published:
Ozempic (semaglutide)
- 0.25 mg pen (starting dose): 120 to 150 euros a month.
- 0.5 mg pen: 120 to 180 euros a month.
- 1 mg pen: 120 to 200 euros a month.
Wegovy (higher-dose semaglutide)
- Depending on the dose: 200 to 320 euros a month. Not funded by the Spanish public health system.
Mounjaro (tirzepatide)
- 2.5 mg pen (starting dose): 180 to 220 euros a month.
- 5 mg pen: 200 to 250 euros a month.
- 7.5 mg to 15 mg pen: 220 to 300 euros a month.
These prices may vary slightly between pharmacies and according to the specific presentation. In cases of type 2 diabetes that meet the funding criteria, the patient's contribution for Ozempic or Trulicity is much lower.
What to look for in a serious medical service
If you decide to access treatment privately (whether face to face or by telemedicine), it is worth applying rigorous criteria when choosing the medical service. The key indicators of legitimacy and quality are:
An identified, verifiable prescribing doctor
The service should clearly show who will be prescribing, with full name, specialty and medical council registration number that can be verified in the register of the relevant Spanish medical council (Colegio Oficial de Médicos).
A relevant specialty
Ideally: endocrinology, preventive medicine, internal medicine or general practice with specific training in obesity. Be wary of services that do not state the prescriber's specialty, or where you only find vague profiles such as "our medical team" with no identifiable professional.
A genuine clinical assessment
The prescribing process must include a genuine clinical assessment: full history, review of recent blood tests, review of contraindications, a consultation with the doctor (in person or by video call), an explanation of the options and informed consent. If a service issues a prescription after a five-minute questionnaire with no real medical interaction, it is operating irregularly.
An official electronic prescription
The prescription must be issued through the private electronic prescription system with full legal validity in Spain, clearly identifying the prescribing doctor, the patient, the medicine, the dose and the instructions.
Dispensing at an authorised pharmacy
The medicine is collected from a legally authorised community pharmacy, not from "warehouses", "clinics" or any other unofficial channel.
Ongoing clinical follow-up
A serious service includes ongoing medical follow-up to adjust the dose, assess response, manage side effects and adapt the plan. This is not a one-off transaction, it is a doctor-patient relationship.
Accessible support
The option to get questions answered between appointments, whether by email, phone, chat or dedicated digital tools. Treatment with GLP-1 agonists throws up questions practically every week, especially at the start.
Transparent pricing
Clear, itemised costs, with no small print or hidden charges. A refund policy if you are not a suitable candidate.
Warning signs of fraudulent or irregular services
- They do not identify the prescribing doctor.
- They do not let you verify a medical council registration number.
- They promise guaranteed access to the medicine with no real assessment.
- They offer aggressive discounts or prices far below the market.
- They make no mention of contraindications and do not ask for a medical history.
- They use stock photos or generic "doctors" with no specific identification.
- Their reviews are suspiciously uniform.
- They operate from odd domains or have changed URL several times recently.
- They ask for payment by bank transfer or cryptocurrency with no official invoice.
Conclusion
Accessing Ozempic or Mounjaro legally and safely in Spain is perfectly possible, but it means going through a real medical process. They are not consumer products and they are not sold without a prescription: they are prescription-only medicines that require an individualised clinical assessment and ongoing medical follow-up.
There are three legal routes: the public health system for diabetes meeting specific criteria, a private face-to-face appointment, or telemedicine with a doctor registered with a Spanish medical council. Everything else (online sales without a prescription, irregular importing, unauthorised compounded formulations, resale between individuals) is illegal and dangerous.
Choosing the right medical service is not a technicality, it is a decision that directly affects your health. Apply the rigorous criteria we have set out and be wary of shortcuts that look too easy.

Frequently asked questions
Can I buy Ozempic or Mounjaro without a prescription in Spain?
No. Selling either medicine without a medical prescription is illegal in Spain. Any website offering them without a prescription is operating outside the law, and the product may be counterfeit or badly stored.
Is telemedicine legal for prescribing Ozempic or Mounjaro in Spain?
Yes, provided the consultation is carried out by a doctor registered with a Spanish medical council with a verifiable registration number, that it includes a genuine clinical assessment, and that the prescription is issued through the private electronic prescription system with full legal validity.
Can they be bought from Spanish online pharmacies?
Online pharmacies authorised in Spain may only sell medicines that do not require a prescription. Selling prescription medicines over the internet is prohibited under Spanish Law 10/2013, although the prescription itself can be transmitted electronically.
What happens if I buy from a website promising Ozempic without a prescription?
You risk receiving a product that is counterfeit, adulterated, badly stored or obtained through illegal channels. On top of that, you are acting outside the law. The AEMPS has issued several alerts about counterfeits in circulation.
How much does the full treatment really cost?
It depends on the route of access and the drug. Going private, you need to add up the initial medical appointment, follow-up appointments and the monthly cost of the medication at the pharmacy. With integrated telemedicine services, the total monthly cost usually ranges between 200 and 400 euros, including the consultation, follow-up and medication.
